Youth Agreement

While participating in this trip, I will accept responsibility for maintaining good conduct. I will comply with chaperones and staff at all times.

Parent Agreement

I give my permission for my child to participate in this activity. To the extent permitted by law, I release City of Refuge Ministries and CORE Community Development, its agents and volunteers from liability for personal injury, property loss, or other damages arising out of this child's participation in this activity.
